Uncertainity: Recurrent abdominal pain in 23 years old women associated with the diagnosis of recurrent renal stones

Patient's problem in her narratives:

I am a 23 years old women

1.    In 2012, suddenly I started to have abdominal pain and after investigations doctors found that I have renal stone. One month after taking medicines and water, the stone removed out from my body.
2.    In 2015, again I experienced an abdominal pain and after investigation again doctors found renal stone. That time also after taking one month of medicines and water, the stone had removed out from my body.
3.    In 2016, this time I was pregnant and due to burning micturition and abdominal pain I had been investigated which revealed 3 stones in both kidneys.
4.    In 2017, at the beginning and in the month of April, I had a mild pain when it again revealed 2 renal stones (both at early 2017 and in April) and it also resolved by taking water for some days. During this period I couldn’t take medicines as I was pregnant and had been breast feeding.
5.    In December 2017 (Few days back), after mild abdominal pain, there was 4 renal stones.

So, the journey of renal stones started from 2012 which used to be coming recurrently and resolving with taking medicines and water.

A 55 year old man with childhood asthma and Acute exacerbation of COPD

This is a 55 year old man in ICU with severe hypercapnia refractory to positive pressure ventilation with PCO2 ranging from 100-120 mm of Hg after 24 hours of ventilation. He appeared to have a history of Bronchial asthma since childhood. Perhaps for the last 45 years. His last three years appear to have been spent in perennial shortness of breath and wheeze. He has a barrel shaped chest and his CXR pa shows pushed down diaphragms. He was referred from a nursing home on a ventilator ambulance and his ABG during admission was showing a PCO2 of 90. After 24 hours of ACMV with a RR of 20 and tidal volumes of 400 ml his PCO2 increased to 120s.  This case of AECOPD with resp. Acidosis on mechanical ventilator was found to have total WBC count increasing overtime (15300 (ICU 2nd Day)/ 18500/ 27500/ 35600 (13th day). So, we started empirically with Clavum & Augmentin and later on planned for meropenam & Colistin antibiotic based on further deterioration in Total WBC Count....
