8 year old boy with speach impairement suspected to have mild autism

This is a 8-years-old boy who has speach impairement, unable to communicate and low eye contact. From the first year of age, he started talking slowly and progressively able to speak well, sing a song, could talk over phone. When he was 3 years old, he had been locked into a room for over 30 minutes and was speaking so loudly from inside adressing his father and finally his father was able to broke the doors and he was so scared while hugging his father. He used to even play crickets and enjoy interacting with people till the age of 4 when his social interaction was gradually decreasing, started geting angry and not able to speak. He cannot tolerate cow voice, ballon and any loud sounds. He had low birth weight (according to his father) and was born 20 days prior to the normal delivery date. At the age 1.5 years, he had a diarrhea for 1-2 days for which doctor provided him some antibiotic injections. 

Parents are planning to take the patient to India all the way from bangladesh and this is very difficult for the patient to travel such a long distance.

57 Year Old female with goiter and thyrotoxicosis

This 57 year olf lafy presented with neck swelling, anxiety and tremor. Her medical information is attached below: We need to take a decision on this lady. To operate or not to operate. Now that our clinical suspicions of thyroticosis have been proved and malignancy not proved (not disproved either) what would be the next best step? Medical management for her thyrotoxicosis or still get an excision biopsy with sub total thyroidectomy (and subtotal thyroidectomy other than disproving malignancy will also become treatment for her hyperthyroidism). So which one should we prefer (if the family has no reservations from their side)?

60 year old woman with hypothyroidism and SIADH

This patient is a 60 year old woman with recent vomiting, coma (sodium 107) and clinical features of dull apathy, skin coarsening, bradycardia, areflexia strongly suggesting hypothyroidism. Serum osmolality and urine sodium are suggesting SIADH. Thyroid profile: Our patient's current dose of sodium is 10ml per hour and she is having mild hypotension at times and in her blood sugar recordings hypoglycemia (attached below) was noted.
