                                                              Patient name - oxalis corniculate   Sex - female age - 60 hight - 5.2 ft. Weight - 52. Bali - 70 cm. Cheast - 85 cm. B.p. - 130/ 80. Suger - 105 after food Sir   আমি একটি রোগীর বিবরণ পাটালাম ১.ঘুম ভাল হয় না, বিশেষ করে রাত্রে প্রায়ই ঘুম হয় না ২. রাতকানা রোগ আছে | কিছু রেটিনা নষ্ট হয়ে গেছে |  দৃষ্টি শক্তি আস্তে আস্তে কমে যাচ্ছে ৩. মুখেরভিতরে ঘা আছে.  এটা প্রায়ই দেখা দেয়. ৪ . পিপাসা মোটামুটি ৫ . চিন্তা গ্রস্থ অবস্থায় প্রায়ই থাকে.  এতে রোগের উপসর্গ গুলি বেড়ে যায় ৬. আগে বুকের বাম পার্ষে প্রায় হঠাৎ খোচা দিয়ে বেদনা হত ৭ . হজম শক্তি কম.  মশলা যুক্ত খাবার সহ্য হয় না.  ৮ . রক্তচাপ মধ্যে মধ্যে বেড়ে যায়.  মাস ছয় আগে রক্তচাপ ১৮০/ ৮০ হয়েছিল.  এখন ১৩০/ ৮০ ৯ . অত্যধিক ঠান্ডা বা গরম কোনটাই সহ্য হয় না ১০ . ঝাল জাতীয় খাবার সহ্য হয়না ১১ . তলপেটের মাঝখানে ব্যথা হয় যা চাপে বেড়ে যায় . কখনো পাশ্ব পরিবর্তন করে ১২ . সায়েটি ক sciatica pain ব্যথায় আগে খুবই কষ্ট পাইয়াছিল.  বতর্মান এ কম তবে প্রায়ই ঠাণ্ডায় তা বেড়ে যায় বাম পাশে ১৩ . স্বাস্থ্য আগে ভাল ছ

A 31 year old woman with chronic Hemolytic anemia and pancytopenia since 10 years and hypothyroidism since 6 months with current admission for severe anemia. There is a pansystolic murmur in  mitral and tricuspid area as well as base of the heart (assumed to be an unusual hemic murmur) and there is no obvious organomegaly. She has sparse hair and generalized pallor with bipedal edema. I have a couple of doubts- Could she have autoimmune disorders considering hemolytic anemia and hypothyroidism. Is it due to Hashimoto ? Was any workup for sle done ?
