A 50-years-old female presented with bilateral leg swelling, chest pain & shortness of breath, having Hb 2.9 gm/dl drammatically improved only with 1.5 months Iron supplements

Journey of a patient from getting typical symptoms of "Anemia with Hb 2.9 gm/dl" to " Hb 11.1 gm/dl with improvement in symptoms" within one and half months:

Story before 1.5 months:

Patient was presented to OPD with the complain of chest pain, shortness of breath and bilateral leg swelling with no history of blood loss. Patient was reasonably well 7 days prior to presentation. ↧

                                                 

Lab investigations were ordered that turned out to have very low HB level (2.9 gm/dl) [All the reports are attached]

Patient was diagnosed with Iron deficiency anemia and prescribed with ferrous fumarate 150mg (OD) for 15 days and no blood transfusions were advised.

After 15 Days:  

Patient came to OPD for the follow up and tests were repeated which turned out that patients HB level is increased upto 5.1 gm/dl. [All the reports are attached]

This time patient was advised to continue the same Iron supplements for 3 weeks more.

1 Month Later:


Patient again came for the follow up and tests were repeated which showed that patients HB level has increased now to 11.1 gm/dl. 





Now, the question remains to answer:

1. How dramatic it is to see Hb level from 2.9  - 5.5 - 11.1 (gm/dl) within 1.5months of iron supplements without blood transfusion?

2. Could it be more better with blood transfusion?

A 55 year old man with childhood asthma and Acute exacerbation of COPD

This is a 55 year old man in ICU with severe hypercapnia refractory to positive pressure ventilation with PCO2 ranging from 100-120 mm of Hg after 24 hours of ventilation. He appeared to have a history of Bronchial asthma since childhood. Perhaps for the last 45 years. His last three years appear to have been spent in perennial shortness of breath and wheeze. He has a barrel shaped chest and his CXR pa shows pushed down diaphragms. He was referred from a nursing home on a ventilator ambulance and his ABG during admission was showing a PCO2 of 90. After 24 hours of ACMV with a RR of 20 and tidal volumes of 400 ml his PCO2 increased to 120s.  This case of AECOPD with resp. Acidosis on mechanical ventilator was found to have total WBC count increasing overtime (15300 (ICU 2nd Day)/ 18500/ 27500/ 35600 (13th day). So, we started empirically with Clavum & Augmentin and later on planned for meropenam & Colistin antibiotic based on further deterioration in Total WBC Count....
